Where did you get your breast augmentation?

I am considering breast augmentation, I would like info on where you received surgery, cost, and if you were happy with the results. (I would greatly appreciate dr.'s name! If you have it). Ohhhh and if you can how did you get the surgery, by bellybutton or by side opening? Details please! I don't know where to start to look! THANK you!
